Empower Youth Coordinator


ANNUAL SALARY: $45,000

Position Summary

The Youth Coordinator is responsible for the successful transition of youth from enrollment in the program to graduation from the program.  The Youth Coordinator provides professional and empowering services to youth, helping them to develop confidence, peer relationships and technical and transferable skills.  The Youth Coordinator will also work with other community and governmental agencies as needed linking students to needed services.  The Youth Coordinator must be an active liaison for youth, as well as possess a demonstrated ability to network with outside agencies to meet needs and promote opportunities provided by the community and Peaceprints of WNY.

 

Essential Functions

  • Networks, establishes and maintains relationships with community service agencies, and government entities.
  • Develop and implement a case management plan for each youth based on individual needs.
    • Testing as appropriate to determine interests, skills and appropriate placement
    • Works closely with youth to identify therapeutic needs, educational/vocational goals, job readiness, employments barriers and available resources
    • Makes referrals and maintains documentation of referrals, benchmark attainments, level of participation and termination/completion
    • Monitors youth progress and modifies service plans as necessary
    • Tracks progress and attendance, and initiates communication with youth and the appropriate service providers to identify and address potential challenges
  • Support the retention process for youth in the program with regular case management and reassessment of plans as needs change.
  • Provide material- written and visual – for organization newsletters and mailings.
  • Manage performance outcomes for students and submit reports on a regular basis.
  • Networks, establishes and maintains relationships with community service agencies, and government entities.
  • All other duties as assigned.

 

Education and Experience

  • Minimum Bachelor’s degree in Human Service, Social Service or another related field.
  • Minimum of 2 years’ experience in education, social, or human services with additional experience in case management.

 

Knowledge, Skills and Abilities

  • Appreciation and understanding of Peaceprints of WNY’s mission and vision, and the ability to effectively communicate the priorities of the campaign.Knowledge of criminal and social justice a plus.
  • Ability to work nights and weekends.
  • Working knowledge of MS Word, Excel, Power Point, and Internet.
  • Preferred Bilingual (English/Spanish).

  • Work time may be evenly split between office settings or outside meetings.Limited local travel is required.Light physical effort in an office environment

  • Regularly required to speak clearly and hear the spoken word as well
